About the Role
SUMMARY: The Client Case Manager will assist in providing case management, enrollment oversight, and initial suicide risk assessments of applicants and clients seeking services through Paws and Stripes.
Requirements
Assists in providing case management for applicants and clients
Measures of Success
A. Assists in assessment/linkage and referral of individuals in crisis to an outside provider (VA, in-patient, etc.)
B. Assessments, referrals, advocacy, developing treatment plans, and documentation as needed.
C. Conducting Psychosocial assessments, evaluation of environmental needs, and determining basic needs that need to be met.
Provides crisis intervention services for veterans and loved ones in the community.
Measures of Success
A. Assessing clients at regular intervals by means of varies measures and assessments and in collaboration with providers on continued need for suicide prevention services.
B. Advising counselors and social workers of clients that are deemed high-risk.
C. Providing education in the community on risk factors, warning signs, suicide prevention services, Veteran's Crisis Line, and treatment options for at risk veterans.
D. Maintaining awareness of suicide treatment options and making recommendations to the patient's provider when new options become available.
E. Conduct suicide prevention psychoeducation groups.
F. Working with community resources such as Veteran Service Organizations and other community groups to assure that they are aware of how to recognize veterans at risk for suicide and their options for getting these veterans assistance.

Measures of Success
A. Training program with non-clinical frontline staff such as clerks and telephone operators to assure that they are aware of options for patients with whom they come in contact during crisis situations.
B. Professional knowledge of the principles and theoretical concepts of social work.
C. Provides clients with information and guidance on available government support programs; conducts follow-up contacts to ensure services are provided.
D. Working collaboratively with treatment team members, attending treatment team meetings to provide recommendations for high-risk veterans.
E. Serve as a point of contact for high-risk veterans not otherwise engaged in care/treatment.
F. Works with Programs Research Manager and other members of the team with VA Grant management, paperwork, and data collection and management.
Formulating treatment goals that address identified needs, stressors, and problems.
Measures of Success
A. Provide crisis intervention, safety planning, case management.
B. Collaborates with supervisor to identify additional available community services.
C. Maintains client case management notes.
D. Contacts clients to arrange appointments.
E. Arranges and participates in multidisciplinary team meetings.
Other duties as assigned
Education and Experience:
Bachelor’s degree in Social Work preferred and experience working with the mentally ill, those with intellectual disabilities, and/or substance abuse.
Must possess and maintain a valid Driver’s License
Excellent verbal and written communication skills.
Ability to work as a leader and as part of a team.
Independent self-starter with positive problem-solver attitude
Computer literate (Word, Publisher, Excel), able to learn & use client care software
Ability to handle multiple priorities, keep organized, and maintain careful attention to detail
Required Skills/Abilities:
Excellent verbal and written communication skills.
Excellent observational and listening skills.
Excellent knowledge of case management and crisis intervention services.
Patient demeanor with the ability to relate to, empathize with, and show compassion for a variety of personalities.
Proficient with Microsoft Office Suite or related software.
Physical Demands and Work Environment:
Work is performed in a highly active environment with exposure to a wide variety of challenges, deadlines and diverse employee and client population
Required to use personal vehicle to travel to potential dog selections, training, solicitations, or other community engagement activities
May be required to handle dogs, with or without training, from time to time as needed
Daily exposure to dogs in the office environment should be expected.
Position may require the occasional lifting of up to 30 pounds.

About the Company
Paws and Stripes, an Albuquerque-based nonprofit, offers animal-assisted therapy and other mental health services to military veterans and their loved ones living with service-connected trauma, including PTSD, brain injuries, and other disabilities. The core of the program is pairing shelter dogs with veterans to be trained as service dogs, while integrating mental health services.
